regarding the collection, use, and disclosure of their personal information by businesses.
As a California resident, you have the right to:
(i) Know what personal information is collected: You can request information about the
categories and specific pieces of personal information a business has collected about you,

(ii) Delete personal information: You can request the deletion of personal information that a
business has collected from you.

(iii) Opt-out of the sale of personal information: You can opt-out of the sale of your personal
information to third parties.

(iv) Non-discrimination: You have the right not to be discriminated against for exercising your
CCPA rights.

To submit a CCPA request, you can either: (1) submit an online request by clicking the hyperlink on the facility website [link to facility CPRA form]; or (2) calling 1-844-440-0098 to submit a request by phone. Clearly
state the nature of your request and provide the necessary information for verification.
When submitting a CCPA request, you typically need to provide:
(i) Your name and contact information.
(ii) Enough information for the business to verify your identity.
(iii) Details about the specific rights you are exercising (e.g., access, deletion, opt-out).
Under the CCPA, businesses generally have 45 days to respond to a verifiable consumer request.
However, they may extend this period by an additional 45 days under certain circumstances.
